Apectomy

An apicotomy is an oral surgical procedure performed on the tip of the tooth root (apex) to remove infection or inflammation in the surrounding tissue. The main purpose of an apicotomy is to preserve a tooth that would otherwise need to be extracted due to persistent infection or the presence of a cyst.

Description of the procedure

The main purpose of an apicotomy is to save a tooth that would otherwise be extracted due to persistent dental infections. Proper preparation, execution of the procedure, and postoperative care are key to a successful outcome. Patients should carefully follow their doctor’s instructions to ensure quick and painless healing.

Candidates

This procedure is indicated for patients diagnosed with:

  • Persistent infection, i.e. when root canal therapy (endodontic treatment) was not successful and the infection recurs.
  • Cyst or granuloma at the tip of the root.
  • A root fracture involving the tip of the root.
  • A root canal perforation that cannot be closed by endodontic treatment.

Preparation

At the dental examination, it is necessary to take a CBCT scan, with which the dentist will confirm the need for an apicotomy and determine the condition of the tooth and the surrounding tissue.

It is possible that the doctor will recommend taking antibiotics before the procedure to reduce the risk of infection.

Procedure progress

Apicoectomy begins with the administration of local anesthesia, so the procedure itself is completely painless for the patient. An incision is made in the gum to allow access to the bone and the tip of the tooth root. The surgeon removes a few millimeters of the root tip along with the infected tissue around it. The area is thoroughly cleaned and disinfected to eliminate the infection, and the root canal is retrogradely filled. The gums are sutured at the end of the procedure to ensure proper wound healing.

Recovery

Recovery after apicotomy lasts 2-3 days, and during this period the following are possible:

  • Mild to moderate pain that can be controlled with analgesics, and your oral surgeon will prescribe the necessary dosage.
  • Swelling is normal after the procedure and may last for a few days. Cold compresses can help reduce swelling.

Maintaining good oral hygiene to prevent infections. It is important to maintain oral hygiene with toothpaste and a toothbrush, taking care not to damage the wound.

It is necessary to remove plaque from the stitches daily with a swab dipped in oral antiseptic.

Do not rinse your mouth with water for the first 24 hours after the procedure, and after that, if there is no bleeding, rinse your mouth with the solution every four hours (without shaking). You can prepare the solution yourself by mixing one tablespoon of salt with one liter of water, you can make sage tea, or you can rinse your mouth with an oral antiseptic based on chlorhexidine.

Precautions

Soft foods and neutral temperature liquids are recommended until the wound heals.

Avoid exercise and intense physical exertion for three days after the procedure.

Do not smoke for 5 days after the procedure and as long as there is bleeding. It is recommended that you do not smoke for the next month.

A check-up is required after 7-10 days to check the healing progress and remove the stitches. Earlier check-ups may be required as needed or at the suggestion of the oral surgeon.
