Nose surgery (rhinoplasty)

Nose correction, also known as rhinoplasty, is a procedure that solves aesthetic and functional problems of the nose, such as a protruding nasal septum, humped nose or deviated septum.

In addition to the external deformation, there are also deformations of the internal structures of the nose that cause difficulty breathing.

During the procedure, the maxillofacial surgeon at the Bagatin Polyclinic tries to solve both problems.

Description of the procedure

There is no single method that can correct or straighten every nose.

Rhinoplasty is one of the most demanding procedures in head and neck plastic surgery.

Knowing different methods and using them, considering the specific problems of each individual, only leads to achieving optimal results: good functionality of the nose, which ensures normal breathing, a harmonious appearance of the entire face, and satisfaction of the operated person.

Today, the approach to rhinoplasty is different than it was before, and the nose is not viewed in isolation, but rather attempts are made to bring it into an optimal relationship with existing facial structures, such as the cheeks, lips, chin, and forehead.

Considering all of the above, the goal of cosmetic nose surgery at the Bagatin Polyclinic is not to get a “new face”, but to reshape the nose to fit it into normal relationships with other parts of the face.

There is a wide range of problems that can be successfully resolved during nose surgery, including: a hump on the dorsum, a crooked, too long or too short nose, a wide nasal pyramid, an irregular nasal tip, a nose that is too wide or extremely narrow, asymmetrical or too wide nostrils, etc.

Surgery can shape both the bone and cartilage structures of the nose.

What you should always keep in mind is that the nose is already partly determined by its initial shape and dimensions.

Overly radical reduction of nasal structures is not recommended because it can weaken the fundamental support of the nose itself.

The firmness and thickness of the skin, as well as the soft tissue covering, have a significant effect on the outcome of the surgery, which also limit the extent to which the nose can be changed.

Although the skin cover does not change, after the operation the skin must cover the previously formed cartilages, therefore excessive reduction of cartilages and bones can result in inadequate adaptation of the skin.

This is especially a problem for people with very thick and oily skin.

Candidates

It is very important to have realistic expectations and be a positive person.

During the consultation, insight is gained into the motivation for surgery, the structures of the nose are analyzed, and the possibilities to what extent certain corrections can be achieved are examined, and only based on all the information obtained is the surgery planned.

During the examination, in agreement with the patient, an individual plan for the surgical procedure is made.

You are a good candidate:

  • if you are looking for an improvement in appearance but not perfection, you are physically healthy, mentally stable and realistic about the results of the operation;
  • if you are dissatisfied with the size or shape of your nose;
  • if you have had an injury or burn that has caused a change in the appearance of your nose;
  • if you were born with a congenital malformation or deformity of the nose (cleft lip and/or palate) and need secondary correction;
  • if you have problems breathing through your nose;
  • if you have completed growth and development, at least 17 years of age, or earlier if it is a congenital malformation or deformity such as cleft lip, nose and/or palate.

Preparation

During your consultation, Dr. Bagatin will give you all the necessary instructions for the surgery: what tests and laboratory results are needed, what to avoid before the surgery (such as alcoholic beverages, smoking, vitamins, etc.), and how to behave after the surgery for the fastest possible recovery.

Before cosmetic nose correction, you should also stop taking painkillers from the group of non-steroidal anti-inflammatory drugs that contain acetylsalicylic acid, such as Aspirin, Andol or Acisal.

This is also one of the most important factors in preparing the patient for surgery because in this way we prevent any unwanted bleeding and hematomas (blood clots) during and after surgery, which significantly speeds up and accelerates recovery and improves the outcome of the surgery.

With the finished findings, an appointment is made for a preoperative examination with Dr. Bagatin and examination by our anesthesiologist.

To be safe, at the final appointment for the surgery, you will determine everything necessary regarding preoperative preparation, surgery, and postoperative course.

Before the consultation, patients should have certain ideas about how they want their nose to look.

During the consultation, you should discuss your expectations in detail.

Based on this, at the Bagatin Polyclinic, we will perform an examination with the VECTRA 3D device, which allows for a realistic display of a 3D simulation on the face, meaning that it gives us the opportunity to show patients how they might look after the procedure.

Procedure progress

The surgery is performed under general anesthesia, which means you are asleep during the procedure.

Aesthetic and/or functional nose surgery itself takes 2 to 4 hours.

At the Bagatin Polyclinic, the “open method” is used for nose correction because this approach gives the surgeon direct inspection and access to the cartilaginous and bony structures of the nose.

This approach allows for far greater precision, improves results and patient satisfaction. It also provides the opportunity for the best results for both minor and major nose corrections.

In the open approach, the incision is made through the columella (the skin of the nasal septum) in the shape of the letter Z and into the nostrils themselves, inside, where it follows the front edge of the cartilage.

The scar on the columella is in the shape of a small step and is almost invisible in the postoperative period.

During the surgery, the bone and cartilage of the nose are reduced or, if necessary, shaped and restored, so that the nose has a satisfactory shape, appearance and size.

Usually, nasal cartilage (most often the nasal septum), ear cartilage, or ribs are used to shape the nose if the aforementioned cartilage is insufficient (previously used) or if a larger reconstruction is needed.

Depending on the problem, Dr. Bagatin, in consultation with the patient, selects the best surgical method.

Recovery

For the first 24 hours, the nose and face appear swollen, with bruises (hematomas), and the swelling increases for up to 48 hours and is expected to peak after 48-72 hours, when a slight headache is also possible.

The nose usually does not hurt after surgery.

If mild postoperative pain occurs, analgesics are usually used in the initial postoperative period.

We recommend not blowing or blowing your nose for the first 2-4 weeks.

It is necessary to avoid sneezing through the nose.

Strenuous physical activity should be avoided for 4-6 weeks after surgery.

It is imperative to avoid sun exposure (use UV factor 50 and a hat), excessive exertion, and any risk of injury.

If you wear glasses, you must make sure that they do not rest on the bridge of your nose for at least a month after the surgery.

Straps and other devices will help you wear your glasses without irritating the area that was operated on.

Recovery is accelerated by cold compresses during the first 72 hours and an elevated head (two pillows), day and night, for the first week after surgery.

After the surgery, we place an external fixator on the nose, and silicone plates in the nostrils, which contribute to protection and facilitate healing of the mucous membrane.

Also, these plates are important in preventing bleeding and supporting the new shape of the nose from the inside.

They will be removed on average after a week.

However, swelling will still be present for several weeks after surgery.

The final result can be expected 12 months after the surgery.

Usually seven to ten days after surgery, the patient is able to return to their daily private and professional activities, with the caveat that they follow all instructions (avoid heavy physical exertion and contact sports, as nasal injury in the early and later postoperative course can cause a new nasal deformity).

Precautions

As with other surgeries, certain complications (inflammation, bleeding) are possible, which Dr. Bagatin will inform you about during your consultation.

By following the instructions provided, the risk of the above is reduced.

The possibility of minor irregularities and asymmetries in the nose during the postoperative period cannot be ruled out, which usually disappear within a year of the procedure.

If a minor secondary correction is needed, it can only be done after at least a year from the first surgery.

Price

The package includes: preoperative examination by an anesthesiologist, surgery under general anesthesia, a short stay at the Bagatin Polyclinic, necessary medications (antibiotics, analgesics), check-ups and dressings in the first postoperative days, removal of stitches, and necessary check-ups up to 3 months after surgery.

In the final phase of recovery, although most of the external changes will be visible, internal structures may continue to adapt and form to their final form several months after surgery.
